It asks service questions
The questions exist so USA OPS can route and service the case correctly.
Single-owner cost
The right question is not “what is the average price?” The right question is whether your specific business can move from an individual path into provider pricing through the PEO structure.
Run the number first. If it points positive, the next step is the underwriter questionnaire and direct provider pricing. If it does not, you stop.
The PEO can service businesses with employees too. USA OPS is here because the owner with no current employees needs a clean way into that review.
Owners usually want the pill: tell me if this can help me, and do not make me become a healthcare expert first.
That answer depends on your income, state, entity, current cost, renewal pressure, and whether the PEO can service the case. A public cost list would be somebody else's answer.
USA OPS turns those facts into a simple next step: stay put, or move to provider review.
Calculator
The questions exist so USA OPS can route and service the case correctly.
A positive result means the case is worth provider pricing and underwriting review.
If the structure is unlikely to help, the answer should say so before a sales call.
The calculator is not the final decision. It is the routing step before the underwriter questionnaire, provider pricing, and start-date work.
USA OPS helps move the file from calculator result to provider review. That includes the underwriter questionnaire, the provider's direct pricing step, and the transition conversation.
If you are on COBRA or a regional individual plan, the work is migration. The provider confirms the final path and start date.
If you add employees later, the PEO can grow with you without changing the basic service relationship that got you started.
For more on the provider path, read the Group-plan access page.
Kept separate
Dental, vision, a Vestwell retirement plan and workers compensation can all run through the same structure. None of them is included in your health number and none is required. If you want them, a short call sorts them out once the health figure works.
We keep them separate deliberately. Bundling them into one figure makes it impossible to tell whether the health decision on its own is a good one.
Limit
The licensed provider makes that call after reviewing the file.
No calculator and no salesperson can commit to final terms beforehand.
What your number tells you is whether applying is worth your time.

Your number takes a few minutes and commits you to nothing.
